（英文）工作經驗證明信格式是什麽？

目前需要給壹名前采購主管開壹封英文的工作證明信，需要寫明其姓名，性別，出生年月日，職位，薪水，工作時間和表現。該主管在我公司工作2年，第壹年是采購助理，後來提升為主管，薪水也有漲。

請問如果開證明信，標準格式是如何的？急，請知情者幫忙答復。
