1 Dla ka?dego uczestnika grupy ? Paszport uczestnika grupy z terminem wa?no?ci przekraczaj?cym 90 dni poza termin wa?no?ci wizy.
針對團組每壹位成員:團組成員的護照在要簽發的簽證有效期後需仍有九十天的有效時間。
For each member of the group ? Passport, valid at least 90 days after expiration of the visa, with a photocopy of the identity page.
2 Dla ka?dego uczestnika grupy ? Wype?niony czytelnie (drukowanymi literami) wniosek wizowy podpisany przez uczestnika grupy z 1 zdj?ciem (35x45 mm, bez nakrycia g?owy i ciemnych okular?w).
針對團組每壹位成員:團組成員需提供字跡工整並貼有壹張照片（35x45mm，免冠並免戴深色眼鏡）的簽證申請表格。
For each member of the group ? One duly completed (in capital letters) visa application form, signed by the applicant, together with recent photo (size 35x45 mm, no head cover, and no dark glasses).
3 Lista uczestnik?w grupy (nazwisko, imiona, data urodzenia, numer paszportu), kopia ID karty oraz hukou ka?dego uczestnika grupy.
團組成員名單（姓，名，出生日，護照號）及每壹位團組成員的身份證復印件。
A name list of all the group members (surname, given names, date of birth, passport number) and copies of ID cards and hukou of each member of the group.
4 Dane personalne szefa grupy (nazwisko, imiona, data urodzenia, numer paszportu, kopia ID karty oraz hukou).
團組組長的個人資料（姓，名，出生日，護照號，身份證復印件）。
Personal data of the tour leader (surname, given names, date of birth, passport number) and copy of tour leader ID card and hukou.
6 Dane dotycz?ce partnera w Polsce (oraz w innych krajach Europy) , obs?uguj?cego lub wsp?pracuj?cego przy przygotowaniach oraz obs?udze grupy w trakcie jej pobytu na terenie RP (nazwa, adres, telefon, fax, e-mail, nazwisko osoby kontaktowej).
協助團組籌備赴波旅遊及團組在波蘭境內期間提供幫助的在波合作夥伴的資料。
Name and contact details of the designated partner tour operator in Poland (and in Europe) ? name, address, phone and fax No, contact name)
7 Szczeg?owy program pobytu grupy w Polsce w j?zyku polskim lub w j?zyku angielskim (w tym m.in. informacja na temat rezerwacji przelot?w grupy na ca?ej trasie Chiny - Europa ? Chiny, dane dotycz?ce hoteli, w kt?rych b?dzie przebywa?a grupa, szczeg?y dotycz?ce poruszania si? pomi?dzy poszczeg?lnymi miastami pobytu na terenie Polski.
波蘭文或英文說明團組在波蘭旅遊的具體日程安排（其中包含團組中國至歐洲至中國全程機票預定信息及團組在波蘭境內主要城市旅遊期間住宿飯店預定信息。
The detailed itinerary (in Polish or in English), including: flight reservation with confirmation for China ? Poland - China round trip from the airline, names, addresses, telephone numbers and fax numbers of the hotels to be used by the group during the tour with the dates of stay in each hotel, and details of how the group is to travel between locations during the tour.
8 Potwierdzenie od partnera z Polski rezerwacji zakwaterowania, wy?ywienia oraz transportu dla ka?dego cz?onka grupy, zgodnie z programem pobytu w Polsce.
在波蘭的合作夥伴根據團組在波蘭旅遊日程為團組成員安排食宿以及交通的證明。
Confirmation from the designated partner tour operator in Poland that accommodation and transport in Poland, as specified in the group?s itinerary, will be provided for all members of the group.
9 Dla ka?dego uczestnika grupy ? Wa?ne ubezpieczenie zdrowotne na ca?y okres pobytu na terytorium RP, zapewniaj?ce pokrycie koszt?w leczenia do kwoty 30.000,00 Euro oraz transportu z Polski do Chin na okoliczno? choroby.
針對團組每壹位成員：需提供在:波蘭境內全程旅行有效的健康保險證明，保額要求達到30000，00歐元以確保支付醫療費和生病的團組成員從波蘭返回中國的交通費用。
For each member of the group ? Relevant insurance covering the trip to Poland, valid for the entire duration of the stay, with cover of at least 30.000,00 Euro and covering the cost of any emergency medical treatment or repatriation for medical reasons from Poland to China.
10 Dla ka?dego uczestnika grupy ? Potwierdzenie posiadania dostatecznej ilo?ci ?rodk?w na pobyt w Polsce ? wyci?g bankowy za okres ostatnich 6 miesi?cy (minimum 300,00 PLN lub 600,00 RMB na ka?dy dzień pobytu w Polsce).
針對每壹位團組成員:需提供申請人在波蘭旅遊期間財務狀況證明即近六個月的銀行存款證明（金額按在波蘭旅遊期間每壹天不少於300，00茲羅提或600，00元人民幣計算）。
For each member of the group ? Proof of financial means ? bank statement for the period of last six (6) months (minimum 300,00 PLN or 600,00 RMB per each day of stay in Poland).

List z zak?adu pracy ? z adresem, telefonem, faxem do biura, podpisem osoby upowa?nionej z piecz?tk? imienn? oraz stanowiskiem - potwierdzaj?ce zatrudnienie oraz stanowisko s?u?bowe, wysoko? wynagrodzenia, zgod? na urlop w okresie wyjazdu turystycznego oraz potwierdzenie przyj?cia do pracy po powrocie z wycieczki.
針對每壹位團組成員:需提供工作單位出具的介紹信（包括：單位地址，辦公室電話及傳真，有簽署權負責人的簽字並加蓋寫明姓名及職務的印章），介紹信需陳述：聘用證明以及申請職人職務及工資，同意申請人休假出國旅遊的證明以及接受申請人旅遊歸來重返工作崗位的證明。
For each member of the group ? A letter of the working unit or company ? with address, telephone and fax of the employer, stamp and signature with name and position of the person signing ? stating: working position, salary, approval for leave and confirmation of working relationship after the trip abroad.

團組從波蘭返回中國後需提供團組所有成員的登機牌復印件以證明團組成員均已離開波蘭***和國國境。
After returning from Poland to China passport of each member of the group for inspection as well as a copy of the boarding cart of each member of the group, proving the fact of leaving Polish territory.
